Welcome to Quality Health Group (“Quality Health Group,” “we,” “us,” or “our”). These Terms and Conditions (“Terms”) govern your use of the Quality Health Group website, www.qualityhealthgroup.org (the “Website”), and your purchase or use of our services.
By accessing our Website, submitting information through our Website, creating an account, or purchasing or using our services, you acknowledge that you have read, understood, and agree to these Terms and Conditions.
If you do not agree with these Terms, please do not use our Website or services.
## 1. SERVICES
Quality Health Group provides medical documentation and medical opinion services that may include, depending upon the services requested and available:
- Nexus Letters / Independent Medical Opinions
- Medical record reviews
- Disability Benefits Questionnaires (DBQs)
- Medical opinions
- C&P examination services
- Virtual and in-office medical consultations or examinations
- Other medical documentation services offered by Quality Health Group
Our medical professionals review information and medical documentation provided to us and, when appropriate, prepare medical opinions or other documentation based upon their professional education, training, experience, clinical judgment, and the evidence available to them.
Not every request will qualify for a favorable medical opinion or Nexus Letter.
Quality Health Group reserves the right to decline a request for services when the available evidence, medical records, or circumstances do not support the requested service.
## 2. NO GUARANTEE OF VA CLAIM OUTCOME
Quality Health Group does **not** guarantee that a Nexus Letter, DBQ, medical opinion, medical examination, or any other service will result in:
- Approval of a VA disability claim;
- An increase in a VA disability rating;
- A particular disability rating;
- Service connection;
- A favorable decision by the Department of Veterans Affairs;
- Approval of an appeal;
- A favorable C&P examination result; or
- Any specific monetary or other VA benefit.
The Department of Veterans Affairs (“VA”) independently evaluates evidence and makes its own decisions regarding disability claims and benefits.
The purchase of a Quality Health Group service should not be interpreted as a promise or representation that the VA will approve a claim or assign a particular disability rating.
## 3. INDEPENDENT MEDICAL OPINIONS
A Nexus Letter or Independent Medical Opinion is a medical opinion prepared by a qualified medical professional based upon the information and evidence available for review.
Our medical professionals are required to exercise their own independent professional judgment.
Quality Health Group does not require, instruct, or direct a medical professional to reach a particular conclusion or provide a favorable opinion.
If the available medical evidence does not support the requested medical opinion, the medical professional may determine that a favorable opinion cannot be provided.
## 4. MEDICAL RECORD REVIEW
Medical record review services are based upon the records and information supplied by the client.
The client is responsible for providing complete and accurate medical records and other documentation requested by Quality Health Group.
Quality Health Group does not have access to the VA's internal medical or claims databases and generally cannot retrieve VA records on behalf of a veteran.
Failure to provide complete or relevant documentation may affect our ability to provide services or may affect the medical professional's ability to reach an opinion.
## 5. CLIENT RESPONSIBILITY FOR INFORMATION
You agree that all information and documentation you provide to Quality Health Group will be accurate, complete, and truthful to the best of your knowledge.
You must not knowingly provide falsified, altered, misleading, or fraudulent medical records or other documentation.
Quality Health Group reserves the right to refuse or discontinue services if we believe that information provided by a client may be inaccurate, fraudulent, altered, or misleading.
## 6. NO LEGAL SERVICES OR REPRESENTATION
Quality Health Group is a medical service provider and does **not** provide legal services or legal representation.
We do not act as your attorney, accredited VA representative, claims agent, or Veterans Service Organization (VSO).
We do not provide legal advice regarding your VA claim, appeal, effective date, evidence strategy, filing strategy, or other legal matters.
You should consult a qualified attorney, VA-accredited representative, claims agent, or VSO for legal or claims-related advice.
## 7. NO DIAGNOSIS OR TREATMENT UNLESS EXPRESSLY PROVIDED
Quality Health Group's medical documentation and medical opinion services are generally intended to evaluate existing medical information and provide medical opinions.
